How to Apply
Providing Hope for Couples Struggling with Infertility
Path to Parenthood
At Pinnacle Fertility, you’ll begin building a relationship with fertility experts in the IVF 100% Success Guaranteed Plan. An application for Samantha’s Gift of Hope can be obtained at Pinnacle Fertility after an initial consultation.
Application Process
Once your completed application is verified by Pinnacle Fertility, it will be forwarded to Samantha’s Gift of Hope to review and make the final decision about recipients.
All completed applications are reviewed and considered regardless of race, religion, ethnicity, or national origin. Please note an award can only be used for services not yet received at Pinnacle Fertility, and funds are given directly to Pinnacle Fertility to cover the expense of the In Vitro Fertilization Success Guaranteed Plan.
Applicants must be a part of Pinnacle Fertility IVF 100% Success Guaranteed Plan in Ohio offices. You must report to Pinnacle Fertility any status changes that occur following the submission of your application. The application sets forth additional and all guidelines and requirements.
